Does Cosmetic Tattoo Hurt? Brows, Lips and Eyeliner Compared

If you’ve been thinking about cosmetic tattoo but keep putting it off because you’re worried it’ll hurt, you’re far from alone. It’s the question we’re asked most often at consultations, usually before anyone asks about colour, shape or price.

The honest answer: most clients find it far more comfortable than they expected. You’ll feel something, but with professional numbing most people describe it as pressure, scratching or vibration rather than pain. How much you feel depends on the area being treated, your skin and a few things you can control on the day.

Here’s what each treatment really feels like, how we keep you comfortable, and what you can do to make your appointment as easy as possible.

The short answer

  • Brows (ombre, nano and combination): most clients rate it around 3 to 5 out of 10. It feels like light scratching or tapping.
  • Lip blush: most clients rate it around 3 out of 10. It feels like vibration or pressure, with the edges and Cupid’s bow a little more sensitive.
  • Eyeliner tattoo: the lash line is the most sensitive of the three areas, so it can feel a little more intense. Numbing is topped up throughout, and the treatment area is small.

Everyone’s pain tolerance is different, so treat these as a guide rather than a promise. If anything feels uncomfortable during your appointment, tell your artist straight away. We can pause, add more numbing or take a break.

What each treatment feels like

Brows: ombre, nano and combination

The brow area has less nerve sensitivity than the lips or eyes, which is why brows are often recommended as a first cosmetic tattoo. Clients usually describe the feeling as a fine scratching or tapping, and some find the sound of the machine more noticeable than the sensation.

The technique matters too. Nano brows use a fine machine needle rather than a manual blade, which most clients find gentler than the dragging feeling some people associate with microblading. We compare the options in Ombre Brows vs Nano Brows vs Microblading.

Lip blush

Lips are naturally sensitive, but numbing works very well on them. Most clients describe a buzzing or vibrating feeling rather than pain, with the outline and Cupid’s bow feeling slightly sharper than the centre of the lips. Lip blush is usually done in passes, with a short rest and more numbing between them. Read more in our complete lip blush guide.

Eyeliner tattoo

The lash line is the most delicate of the three areas, so it’s normal to feel more aware of the treatment here. Your eyes may water, which is a reflex rather than a sign of pain. Numbing cream is applied before we start and reapplied as needed, and the area being worked on is very small. Find out more in What Is an Eyeliner Tattoo?

How we keep you comfortable

  1. Consultation first. We go through your design, your skin and anything that might affect your comfort, including past reactions to numbing creams.
  2. Topical numbing cream. A professional numbing cream is applied and left to work for about 20 minutes before any pigment goes in.
  3. Top-ups as we go. We reapply numbing during the treatment, especially between passes.
  4. Breaks whenever you need them. You’re in control. If you need a moment, just say so.
  5. Constant check-ins. We’d much rather you tell us about a sharp spot in the first 30 seconds than tough it out.

What can make it feel more sensitive

A few everyday factors can make your skin more reactive on the day:

  • Caffeine and alcohol. Both can make you more sensitive and can increase bleeding, which affects how well pigment settles. Skip them on the day and the night before.
  • Blood-thinning medication and painkillers. Some medications can affect bleeding or bruising. Don’t stop prescribed medication or change how you take it without consulting your healthcare professional. Let your artist know about any medication you take before your appointment.
  • Your monthly cycle. Some clients feel more sensitive around their period, so you may prefer to book outside that week.
  • Tiredness, hunger and nerves. A good night’s sleep, a proper meal beforehand and arriving relaxed all make a real difference.
  • Irritated skin. Sunburn, recent peels or retinoid use near the area can make skin more sensitive. We cover what to stop and when in our cosmetic tattoo pre-care guide.

Tips for a comfortable appointment

  • Eat beforehand and bring a water bottle.
  • Wear comfortable clothes and bring headphones if music helps you relax.
  • Breathe slowly through any moments that feel sharper. It really does help.
  • Speak up. Your artist can’t feel what you feel, so a quick “that spot’s a bit sharp” helps us look after you.
  • Plan a quiet afternoon afterwards so you’re not rushing straight back to a busy day.

Does it hurt afterwards?

Not usually. Once the numbing wears off, most clients describe the area as tight, warm or tender rather than painful, a bit like mild sunburn.

  • Brows: a feeling of tightness for the first day or two, then some itching as they heal. See the ombre brow healing timeline.
  • Lips: some swelling and dryness for the first few days.
  • Eyeliner: puffiness around the eyes, usually worst the morning after and easing within a day or two. See our eyeliner tattoo healing guide.

Ongoing or worsening pain, spreading redness, discharge or any change to your vision isn’t normal. Contact us, and see a GP if you’re worried.

Do touch-ups hurt?

Most clients find their perfecting session about the same or slightly more comfortable than the first appointment. Numbing is applied the same way, and we’re refining existing pigment rather than starting from scratch. See Cosmetic Tattoo Touch-Up: When and Why You Need One.

Frequently asked questions

Does eyeliner tattoo hurt?

It’s the most sensitive of the three cosmetic tattoo areas, but most clients find it very manageable with numbing cream, which is topped up throughout. Watery eyes are common and normal.

Which cosmetic tattoo hurts the most?

Eyeliner is usually the most sensitive, followed by lips, with brows the most comfortable for most people. With numbing, all three are well tolerated by most clients.

Is numbing cream used for every treatment?

Yes. A professional topical numbing cream is used for brows, lips and eyeliner, and reapplied during the treatment as needed. Let us know if you’ve ever reacted to a numbing cream.

Can I take a painkiller before my appointment?

Avoid ibuprofen and aspirin, as they thin the blood. Check with your GP or pharmacist if you’re unsure.

Will it hurt more if I have a low pain tolerance?

You might be more aware of the sensation, but that’s exactly what numbing top-ups and breaks are for. Tell us at your consultation so we can plan your appointment around you.

Does cosmetic tattoo hurt more than a normal tattoo?

Most clients say no. Cosmetic tattoo works in the upper layers of the skin with fine needles, and unlike most body tattoos, professional numbing is used throughout.
